New Agent Intake for Course

This Terms of Agreement ("Agreement") is made and entered into by and between Myers Insurance Consulting LLC ("Company") and the undersigned individual.

By signing this Agreement, the Agent agrees to the following terms and conditions related to the 2-40 Health Insurance Agent Pre-licensing Course( or 2-15) and the Florida Department of Insurance licensing requirements. XCEL Solutions is our Insurance Training Provider.  


  • Course Enrollment and Payment 

    • The Company agrees to pay for the Agent's enrollment in the 2-40 Health Insurance Agent Pre-licensing Course (Course ID 109249) under the described requirements:

      • Course Completion and Exam Requirement 

        • The Agent agrees to complete the 2-40 Health Insurance Agent Pre-licensing Course and complete and pass the course exam within 30 days from the date of enrollment and schedule the State test within 2 weeks of passing all course requirements. 

          • Should the Agent fail to complete the course and pass the exam within this period, the Agent will be required to reimburse the company the full cost of the course of $100.00. 

  • Exam Fees 

    • The Agent is responsible for paying the required Department of Insurance/ Financial Services, exam fee for the 2-40 exam, which ranges from $40-$85 per attempt. Payment of the exam fee is required directly to your state. 

    • The intake form and authorized card information provided will be used to secure your course account with Xcel Solutions, state exam, fingerprinting and background providers. All information must match your current/active state identification and be true and correct. 


  • Scheduling the Exam and Fingerprinting Requirements

    • To schedule the exam, the Agent will reserve an appointment at a physical test center approved by their state. 

    • The Agent understands that the Company can assist in securing an appointment. But the Company is not responsible for any delays/issues that may occur in scheduling the exam. It is the Agent’s responsibility to make, keep, and follow up with your exam, fingerprinting, and all necessary appointments. 

      • All states require an account registered to your email on their respective websites, so you will be notified of any and all communications directly, even with our assistance.

  • Eligibility Requirements

    • The Agent must meet the following eligibility criteria to obtain a 2-40 Health Only Insurance License in your state: We can also do other lines of authority, please ask during consult.

      • Have a valid state license.

      • Be at least 18 years old 

      • Be a U.S. citizen or legal alien 

      • Complete the required online application 

      • Submit the required fees

      • Complete fingerprinting at the Department of Insurance for your state

      • Pass a background check

  • Examination and Passing Criteria 

    • To pass the 2-40 Health Insurance Agent exam (or 2-15 upon request), the Agent must achieve a score of 70% or higher. The passing score is valid for one year from the date the exam is passed. If the Agent does not complete the licensing process within one year, they will be required to retake and pass the exam.

    •  The Agent can attempt the same exam up to five (5) times within a 12-month period or based on your state, limitations. 

  • Reimbursement Requirement 

    • If the Agent does not meet the completion or testing requirements outlined in this Agreement, they will be required to repay the Company for the cost of the course in full. 

  • Miscellaneous 

    • This Agreement is the entire understanding between the parties regarding the subject matter here. Any amendments or modifications to this Agreement must be made in writing and signed by both parties. 


By signing below, the Agent acknowledges that they have read, understood, and agreed to the terms and conditions outlined in this Agreement.
